Employees must successfully complete the Defensive Driving Training required by the Office of Risk Management within (90) days of employment, Complete the Driver’s Authorization Form (DA 2054), and print the certificate and forward to the Environmental Safety Department. Once your certificate has been received, your request will be processed and an e-mail will be sent letting you know the if you have been authorized to operate a vehicle on agency/college business.
